First Visit/Initial Appointment
Description
The first visit/ initial appointment includes an assessment. In order for me to provide a treatment that is safe and effective, I will need to determine what is going on with you. The assessment will begin with an interview based on your health history information. Please do well to fill out the intake form sent to you after booking. It is important that we have a complete and accurate health history so that I can create a treatment plan for you that is safe, knowledgeable, and effective.
